Information required to report an infringement

1. Authorized Signature

2. Description of the allegedly infringed work
Provide a detailed description of the work or material whose rights you believe are being infringed.

3. Location of the material
Specify the exact location on our website where the infringing material can be found, e.g., the product URL(s).

4. Contact Information
Provide us with sufficient contact information so that we can reach you, including your name, address, telephone number, and email address.

5. Declaration to the Best of Your Knowledge
Include a statement confirming that, to the best of your knowledge and belief, the use of the material in question is not authorized by the intellectual property rights owner, its agent, or the law.

6. Intellectual Property Rights Identification
Clearly identify the intellectual property rights that you believe are being infringed (e.g., "XYZ Copyright," "ABC Trademark, Registration No. 123456, registered January 1, 2004").

7. Declaration of Accuracy and Authorization
Include a sworn statement confirming that all information provided is accurate and that you are the intellectual property rights owner or authorized to act on its behalf.
